A medication-management resource for your patients

Sarah Rego, PMHNP-BC, is a board-certified psychiatric-mental health nurse practitioner with over 10 years of healthcare experience. Her acute-care background informs a whole-person approach to assessment, individualized medication planning, and ongoing monitoring.

Therapists, primary-care providers, and other clinicians can contact Pineview to discuss whether the practice is an appropriate option for a patient seeking psychiatric medication management. Please confirm patient eligibility, clinical fit, and current availability before planning a transfer.

How to start a referral

  1. Contact the practice. Call (508) 233-8656 to ask about the patient’s care needs, eligibility, and the next steps for referral.
  2. Arrange records through the practice. Ask which records are needed and confirm the appropriate transmission method and any required patient consent before sending clinical information.
  3. Help the patient take the next step. Share the booking and intake links once the referral pathway is clear. Patients should confirm their insurance coverage and expected costs. A referral or completed intake does not itself establish care or confirm an appointment.

Information that may help the assessment

After confirming how to share records, useful information may include the reason for referral, current concerns and treatment goals, relevant diagnoses or assessments, current medications, prior medication trials and responses, pertinent medical history, and the referring clinician’s contact information.

Please do not put patient names, diagnoses, records, or other medical details into ordinary email. Call the practice to arrange the appropriate way to exchange information.

Supporting continuity of care

If the patient is already receiving therapy or medication management, identify the current treating clinicians and discuss what coordination may be needed. Confirm consent and information-sharing arrangements with the practice.

Do not assume Pineview has accepted responsibility for treatment or refills simply because a referral was sent. Confirm the receiving clinician, appointment, and transition plan before ending or transferring existing care.

Routine referrals and urgent needs

This referral pathway is for routine outpatient care. Do not use the intake form or a referral message for emergency response. For crisis support in the United States, call or text 988; call 911 for immediate danger or a medical emergency.
